O maior hospital de segurança do Front Range está com dificuldades para sobreviver e a Questão Eleitoral 2Q injetaria US$ 70 milhões por ano no Denver Health.
O custo de reforçar esse sistema de saúde crítico seria um imposto sobre vendas de 0,34%, o que se traduz em um pouco mais de US$ 0,03 em cada US$ 10 que alguém gasta em itens alimentares não alimentícios na cidade e no condado de Denver. É um investimento que os eleitores desta cidade podem fazer para o futuro da assistência médica.
O Denver Health é há muito tempo um dos melhores hospitais de trauma do estado — o lugar para onde você quer que uma ambulância vá quando uma tragédia acontece — mas ele atende o estado de outras maneiras. A unidade de mães e bebês faz o parto de uma em cada três crianças nascidas em Denver. A entidade quase governamental também opera clínicas de cuidados primários e saúde mental em nossas escolas, bairros e prisões. Ela opera ambulâncias e contrata nossos paramédicos. Ela tem clínicas odontológicas e fornece vacinas.
O hospital operou com prejuízo líquido nos últimos três anos, embora nos últimos dois anos, a nova CEO, Donna Lynne, tenha conseguido apertar o cinto e solicitar fundos de emergência suficientes para permanecer no azul — por pouco. Em 2022, o hospital relatou um prejuízo de US$ 58 milhões e, em 2023, o hospital saiu na frente por US$ 11 milhões.
Os fatores determinantes dessa crise fiscal foram o aumento de custos associados ao aumento de salários e inflação, e o aumento de cuidados não compensados de indivíduos sem-teto, migrantes sul-americanos e os milhares de coloradenses que foram expulsos do Medicaid este ano. O hospital recebe pagamentos estaduais, federais e da cidade de Denver para pacientes que não podem pagar, mas esses pagamentos permaneceram bastante estagnados por muitos anos.
O Denver Health precisa de uma fonte de receita dedicada para que possa continuar sendo uma força próspera para a saúde pública em nossa comunidade.
Nos últimos cinco anos, o conselho editorial do The Denver Post adotou uma postura conservadora em relação aos aumentos de impostos sobre vendas, pedindo aos eleitores que não criassem novos programas usando a capacidade limitada da cidade de aumentar os impostos sobre vendas.
Mas Denver ainda pode arcar com mais um aumento de imposto sobre vendas, e esta proposta não é para um empreendimento novo, não testado e ingovernável. O Denver Health é uma das instituições mais antigas do estado. Em um ponto, era uma agência municipal antes de se tornar independente e historicamente fazia seu orçamento funcionar apertando o cinto em tempos difíceis e expandindo apenas quando prudente.
Donna Lynne tem um histórico comprovado de liderança. Ela comandou o Kaiser Permanente no Colorado antes de se tornar vice-governadora do estado sob John Hickenlooper. Ela prometeu que o hospital divulgará exatamente como a nova receita do imposto sobre vendas é gasta durante seu “Relatório para a Cidade” anual. A linguagem da cédula limita como o dinheiro é gasto a cinco categorias principais de cuidados: emergência, primário, saúde mental, pediátrico e recuperação de abuso de substâncias. Além disso, quando os membros do Conselho Municipal de Denver votaram 12-1 para encaminhar esta medida aos eleitores, eles incluíram uma linguagem estipulando que a contribuição anual de US$ 30 milhões da cidade para o Denver Health não seria reduzida em resposta ao novo imposto, embora outros fatores econômicos pudessem causar uma redução.
A segurança financeira do Denver Health beneficia toda a região. O hospital atende pessoas de todo o estado, não apenas da área metropolitana, então é apropriado que um imposto sobre vendas pago não apenas pelos moradores de Denver, mas também pelos visitantes, seja a fonte de financiamento para as operações.
Parte do dinheiro permitirá a expansão das clínicas móveis do hospital para áreas carentes da cidade, mas a maior parte do novo fluxo de receita dedicado ajudará a evitar demissões, melhorar a retenção de funcionários e evitar possíveis reduções nos serviços.
No entanto, os representantes eleitos do Colorado não podem descansar sobre os louros mesmo que a Questão Eleitoral 2Q seja aprovada. Duas outras grandes reformas são necessárias.
O Colorado cobra uma taxa dos pacientes por cada noite passada em um hospital e usa essa taxa para obter dólares federais correspondentes para pagamentos do Medicaid. Esses dólares correspondentes são então enviados aos hospitais por meio de uma equação opaca que deve ser revisada com base nas necessidades dos hospitais. Se o Colorado Healthcare Affordability and Sustainability Enterprise Board não ajustar a fórmula, os legisladores estaduais devem forçá-los.
Em segundo lugar, os legisladores do Colorado podem reforçar os requisitos para que hospitais “sem fins lucrativos” gastem mais de seus “benefícios comunitários” necessários ajudando outros hospitais e provedores médicos com seus cuidados não compensados. Os hospitais sem fins lucrativos devem ajudar a preencher os cuidados não compensados de suas regiões antes de gastar o dinheiro em outros propósitos. A Kaiser Permanente estabeleceu o precedente para isso em 2023, quando doou US$ 10 milhões para a Denver Health e iniciou um esforço de arrecadação de fundos este ano para a Denver Health Foundation.
O sistema de saúde nos Estados Unidos está quebrado. Tudo é muito caro, enquanto nossos profissionais de saúde geralmente trabalham demais e são mal pagos. Até mesmo os melhores provedores médicos podem ter dificuldades financeiras, e há falhas profundas no Medicaid, Medicare e todas as seguradoras privadas.
Mas uma coisa que podemos consertar, em novembro, é garantir que o Denver Health tenha os recursos necessários para atender Front Range nos próximos anos.
