O câncer de pulmão é a segunda principal causa de morte por câncer nos Estados Unidos, depois do câncer de pele, de acordo com a American Cancer Society. A doença é responsável por cerca de 1 em cada 5 mortes por câncer, e a ACS estima que mais de 125 mil pessoas morrerão por causa dela este ano.
Essas estatísticas são sombrias, mas há uma razão pela qual o câncer de pulmão muitas vezes progride além do ponto em que é tratável: os primeiros sinais de câncer de pulmão muitas vezes passam despercebidos (ou são inexistentes).
“A maioria dos pacientes pode não apresentar sintomas, a menos que tenha se espalhado ou que haja evidências de que seja metastático ou que haja envolvimento de estruturas importantes”, diz a Dra. Amna Sher, médica oncologista do Stony Brook Cancer Center.
O diagnóstico precoce pode melhorar significativamente as chances de sobrevivência de uma pessoa. Por exemplo, a taxa de sobrevivência de cinco anos para o tipo mais comum de cancro do pulmão, o cancro do pulmão de células não pequenas, que inclui o adenocarcinoma, o carcinoma de células escamosas e o carcinoma de células grandes, é de 65% se for diagnosticado enquanto ainda está localizado (por outras palavras , não se espalhou por outras partes do corpo).
Esses números caem para 37% se a doença se espalhar para estruturas próximas aos pulmões, como os gânglios linfáticos, e apenas 9% se se espalhar para áreas mais distantes do corpo, como o cérebro.
Mas o resultado final é o seguinte: “Se o câncer de pulmão puder ser diagnosticado em um estágio inicial, o paciente terá mais opções de tratamento e melhores chances de remissão”, diz o Dr. Xiuning Le, membro da V Foundation. pesquisador clínico do MD Anderson Cancer Center da Universidade do Texas. “É extremamente importante detectar o câncer de pulmão o mais cedo possível.”
Mas como? Os médicos compartilharam os primeiros sinais de câncer de pulmão que costumam passar despercebidos e o que fazer se você estiver preocupado.
Sinal comumente perdido
Uma tosse persistente ou que piora é um sinal precoce de câncer de pulmão comumente esquecido, diz Sher.
“É necessária uma avaliação se alguém tiver uma tosse nova ou persistente… que persista há várias semanas ou meses, especialmente num paciente de alto risco com um forte histórico de tabagismo”, diz ela. “Por exemplo, você tomou antibióticos, mas já se passaram dois meses e você ainda está tossindo – procure atendimento médico.”
Sher diz que os médicos costumam ver pacientes chegando com meses de tosse persistente, mas um raio-X anormal leva a um mergulho mais profundo na causa do problema.
Fácil de descartar
Le e outro especialista concordam que a tosse crônica é um sinal fácil de ignorar de câncer de pulmão. Parte do problema: a tosse crónica é muitas vezes semelhante a outras doenças, incluindo aquelas com maior risco de cancro do pulmão – principalmente os fumadores – que já podem ter.
“A maioria – mas não todos – dos casos de câncer de pulmão está associada a um histórico prévio de tabagismo”, diz o Dr. Brett Bade, diretor do Programa de Rastreio do Cancro do Pulmão do Northwell Lung Insitute, em Nova Iorque. “O tabagismo é um fator de risco tanto para câncer de pulmão quanto para doenças pulmonares. A doença pulmonar obstrutiva crônica, por exemplo, também pode causar tosse crônica e falta de ar. As pessoas podem atribuir seus sintomas à doença pulmonar ou ao uso prévio de tabaco.”
Em outras palavras, os pacientes podem estar acostumados a tossir e considerá-lo normal.
Mesmo pessoas sem histórico de tabagismo ou doença pulmonar podem atribuir uma tosse persistente a outra coisa e, geralmente, a algo menos grave.
“Os pacientes podem pensar que é apenas uma infecção respiratória superior ou um vírus”, diz Sher. “Pode ser sazonal. Os pacientes têm alergias. Se você está perto de pacientes com crianças, você está exposto a todos esses vírus e pode pensar que simplesmente não melhorou.”
Outros sinais esquecidos
Uma tosse persistente não é o único sinal precoce de câncer de pulmão fácil de ignorar.
“Como os pulmões não têm muitos receptores de dor, a maioria dos sintomas do câncer de pulmão está relacionada ao envolvimento do tumor em uma estrutura local ou outro órgão”, diz Bade. “Por exemplo… pode ocorrer falta de ar se o tumor envolver as vias aéreas. Da mesma forma, se o tumor se espalhar para um osso, o osso envolvido fica dolorido.”
Bade diz que outros sinais comuns de câncer de pulmão que um paciente pode não reconhecer incluem: fadiga, rouquidão, dor no peito, dor nos ossos, redução do apetite e perda de peso.
Diagnóstico, tratamento
Os médicos farão uma biópsia do local envolvido para diagnosticar câncer de pulmão.
“Os locais de biópsia frequentes incluem os pulmões, gânglios linfáticos no pescoço ou tórax ou drenagem de líquido ao redor dos pulmões”, explica Bade. “Se outros órgãos estiverem envolvidos, seu médico poderá recomendar uma biópsia desses locais; exemplos podem incluir o fígado, a glândula adrenal ou um osso.”
Le afirma que vários fatores determinam o plano de tratamento do câncer de pulmão de um paciente, principalmente: o tipo de câncer de pulmão, o estágio do câncer e as características genéticas e imunológicas do paciente específico.
“Em geral, cirurgia, radiação e terapias sistêmicas são tratamentos eficazes para o câncer de pulmão”, diz Le. “Existem muitas opções diferentes dentro da aula (de terapia).”
Uma equipe de atendimento trabalhará com o paciente na melhor opção para seu caso específico.
