A próxima divisão dos grupos médicos Graybill e Palomar Health do Condado de North forçará cerca de 45.000 pacientes a tomar uma decisão: seguir seus médicos para diferentes consultórios médicos ou ficar e ser atendido por outra pessoa.
Esse enigma foi motivado pela Graybill, uma respeitada prestadora de cuidados primários desde 1932, que notificou a Palomar em agosto que rescindiria um acordo de serviços de gestão com o distrito de saúde pública, citando “suporte e resposta inadequados às consequências do ataque cibernético de maio de 2024 direcionado ao Palomar Health Medical Group”.
O ataque desligou os sistemas de computador de ambas as organizações, incluindo os serviços de telefonia digital, tornando os registros médicos eletrônicos inacessíveis por meses e forçando a equipe a documentar tudo, desde consultas até receitas, com caneta e papel.
“A violação e a incapacidade de restaurar todas as funções críticas prejudicaram a capacidade do nosso grupo de fornecer serviços médicos essenciais aos nossos pacientes”, disse a Dra. Kelly Boyatt, especialista em medicina de família de Graybill, em uma declaração emitida pela organização.
Os dois lados discordam sobre a extensão em que as consequências da invasão de dados foram resolvidas, com Graybill dizendo que “funções críticas (ainda) não estão totalmente restauradas” e Palomar dizendo que “as operações foram totalmente e com segurança restauradas”.
A Palomar Health respondeu em sua própria declaração na tarde de quarta-feira, negando responsabilidade exclusiva pela resposta ao incidente de violação de dados.
“A Graybill tenta se distanciar das consequências do incidente de segurança de dados, apesar de um dos principais líderes da Graybill atuar como Diretor de Informações Médicas do Palomar Health Medical Group”, disse o comunicado.
Dra. Vanessa Peters, médica chefe da Graybill, disse que a organização tentou manter a divisão amigável. Os 100 médicos e provedores médicos afiliados da Graybill, ela disse, continuarão a encaminhar pacientes para especialistas e hospitais da Palomar em Escondido e Poway.
Mas essas garantias aparentemente não foram suficientes.
“Eles nos notificaram em 10 de setembro com um memorando para todos os funcionários e provedores da Graybill de que a Graybill estava demitindo (e) portanto precisava sair do prédio em 11 de novembro”, disse Peters.
Atualmente, o grupo opera consultórios médicos na Second Avenue e El Norte Parkway em Escondido, Fallbrook, Vista, Murietta, Oceanside, San Marcos, Temecula e Valley Center.
Todos, exceto dois dos nove locais de Graybill, são afetados, disse Peters. Locais em Vista e Oceanside não precisarão se mudar. No geral, o grupo estima que algo entre 91% e 93% de seus 50.000 pacientes terão que seguir seus médicos existentes para novos locais, embora esteja claro que Palomar está oferecendo uma segunda opção.
Palomar discordou da caracterização da situação feita por Graybill como um “despejo”, indicando que os médicos podem escolher permanecer em seus prédios atuais como parte do Palomar Health Medical Group, se desejarem.
“Estamos oferecendo a todos os médicos a oportunidade de permanecer e continuar praticando com a PHMG”, disse Russ Riehl, diretor executivo interino do grupo Palomar. “Não houve despejo de nenhum médico, e continuamos esperançosos de que aqueles que estão pensando em sair entrem em contato para explorar como podem continuar trabalhando conosco.”
A declaração de Palomar acrescenta que “muitos médicos de Graybill entraram em contato para discutir a permanência no PHMG para garantir um atendimento contínuo e contínuo”.
Mas Peters chamou essa declaração de falsa, afirmando que apenas seis especialistas que são membros da Graybill fizeram tais propostas para Palomar. Ela também se manteve firme chamando a situação de despejo, fornecendo uma cópia do memorando geral de 10 de setembro que afirma que os membros da Graybill “deixarão de fornecer serviços nesses locais em 10 de novembro de 2024”, acrescentando que os provedores da Palomar “começarão a atender pacientes nesses locais a partir de 11 de novembro de 2024”.
Como a situação chegou ao ponto em que Graybill se vê sendo forçada a desocupar tantos prédios, alguns dos quais ocupa há décadas? Peters disse que a organização transferiu seus arrendamentos de longo prazo para Palomar quando os grupos concordaram em fazer uma parceria em 2020.
“Normalmente, em uma transação em que as partes estão se separando, um contrato de arrendamento comercial será atribuído de volta à parte que está saindo de forma amigável, e era isso que esperávamos que acontecesse”, disse Peters, acrescentando que os médicos de Graybill agora estão procurando novos locais.
Eric Goldy, um morador do Valley Center que enfrentou o ataque cibernético de meses de duração para fazer uma pequena cirurgia previamente agendada, disse na quarta-feira que sabe como tomará sua decisão. Ele disse que está na Graybill desde antes da fusão com a Palomar e não vê isso mudando tão cedo.
“Eu já segui meu cara para pelo menos três prédios (ou) locais diferentes conforme Graybill se expandia, então, sem dúvida, estou muito mais apegada ao meu cara do que ao meu prédio”, disse Goldy. “Ele é meu médico há provavelmente mais de 20 anos; ele sabe mais sobre minha saúde do que minha esposa.
“Eu vou ficar com ele.”
